Using Forms with 2D Barcodes

Several jurisdictions require 2-D Barcodes on a printed form. These black and white barcodes contain specific encoded data generated from the form's data entries, and are used for electronic or machine processing.

All displayed and printed forms should typically contain 2-D Barcodes. If any mandatory entries are missing from the form, Barcodes will not be generated correctly and the form cannot be filed as displayed and printed forms will reflect these errors.

Orange fields on a form indicate mandatory entries.
